Analysis of EFCOG Electrical Safety Charts for December 2008

SC Program Office did achieve a decreasing trend (greater than 10 of 11 months below average) for 08J Electrical Near Miss.  However, December came in with two reports in the same month.  A decision to baseline the chart to a zero rate is being deferred until more data are available.

NA Program Office developed a decreasing trend for Electrical Events overall.  The past four months have been one standard deviation below the baseline average of 5.5 per month.  A new baseline average and control limits were developed for 08A Electrical Shocks due to its recent decreasing trend.  The new baseline is 0.5 per month, less than one-third of the previous baseline of 1.6 per month.

EM Program Office 08A Electrical Shocks have been set to a zero baseline following previous decreasing trends.

All other charts are stable on their current baselines.

“A3B2C02 - Signs to stop were ignored and step performed incorrectly” has remains in first place as the leading cause of 08J electrical near misses over the past 12 months.  “A4B3C11 - Inadequate work package preparation” is now tied with A3B2C02 for first place.
