Analysis of EFCOG Electrical Safety Charts for February 2008

Most of the indicators were at lower levels than the significant increases for last month (January 2008), but there may be some indicators of non-improving trend continuing.

The total number of EM electrical events was at four for February, which was above the current baseline.  With the point above the Upper Control Limit for January, there may be an increasing trend developing for July 2007 through February 2008.  There were 38 EM reports in eight months as compared to 18 reports in the eight months from November 2006 to June 2006.

The EM chart for 08J Electrical Shocks may be developing a long term trend.  There was one 08J report in February (still above baseline), making seven such reports in the last three months, as compared to 13 months to accumulate the previous seven 08J reports.

There was an increase for 08A Electrical Shocks across the DOE complex for January.  January came in above the Upper Control Limit with six reports.   February was lower, with four reports, but still above the baseline average.

EM 08A Electrical Shocks did return to zero for February.

The SC chart for total Electrical Events has been six months in a row below average.  If March is also below average, that will be a significant improving trend. 

The DOE Complex-wide 08J Electrical Near Miss chart has had a new reduced baseline average and control limit calculated following the significant decreasing trend noted in the past two reports. 

“A4B1C01 - Management policy guidance / expectations not well-defined, understood or enforced” moved into first place as the leading cause of 08J electrical near misses over the past 12 months.
