Analysis of EFCOG Electrical Safety Charts for December 2009

The overall number of Electrical Events reports was close to the Upper Control Limit for September with 18 reports.  The data since have returned closer to baseline, but the past five months have been above average.  Another two months above average will be a significant increasing trend. 

Electrical Shocks (08A) for All DOE have been above average for more than seven months in a row.  This is a significant increasing trend.  A new baseline average and control limits will be developed when the data stabilize.

SC Program Office charts are stable.  Electrical Near Miss (08J) did come in close to the UCL in November, and another month similar to this in January will cause a trend to be declared.

NA Program Office 08A Electrical Shocks have a significant increasing trend with November above the UCL, and both October and November more than two standard deviations above average.  December did return to closer to baseline.  A new baseline average and control limits will be established if the data stabilize higher than the existing baseline.

NA 08J Electrical Near Misses were close to the UCL in November.  Another month similar to this in December or January will cause a trend to be declared.

EM Program Office 08J Electrical Near Miss Chart is one month away from an increasing trend.  The past three of four months have been one standard deviation above average.

All other charts are stable on their current baselines.

Check of Work was Less than Adequate (A3B1C01) remains the leading cause of 08J Near Miss events.  A3B1C03 - Incorrect performance due to mental lapse and A3B2C05 - Situation incorrectly identified or represented resulting in wrong rule used are now tied with A3B1C01 for the leading cause, each with 5 events over the past 12 months.
