DOE Complex Electrical Event Charts
October 2009
11/30/09
Analysis of EFCOG Electrical Safety Charts for
October 2009
The overall number of Electrical Events
reports was close to the Upper Control Limit for September with
18 reports.
October did come in closer to
baseline, with 13 reports, so this has not tripped the criteria
for a trend.
Electrical Shocks (08A) for All DOE have
been above average for six months in a row.
If November has two or more 08A
reports, this will be a significant increasing trend.
SC Program Office charts are stable.
NA Program Office 08A Electrical Shocks
were close to the Upper Control Limit with two reports.
If November or December have two or
more reports, this will be an increasing trend.
NA 08J Electrical Near Misses did
have two reports in October, justifying generating a new
baseline average from the previously noted increasing trend in
early 2009.
The new baseline is 1.0 per month,
up from the previous baseline of 0.6 per month.
EM Program Office 08A Electrical Shocks
have been zero for the past two months, following the
significant spike in August 2009.
Total Electrical Events and 08J
Electrical Near Misses returned to near normal for October,
following their nearly significant increases in September.
All other charts are stable on their
current baselines.
A3B1C03 Incorrect performance due to mental lapse continues to
be a leading cause of events for the past 12 months, although
there are now six causes tied for the leading cause.
