Analysis of EFCOG Electrical Safety Charts
for February 2010
Although January 2010 dropped back below average for 08A
Electrical Shocks across DOE, February 2010 came in at another
elevated level with four reports.
It does appear that the increasing
trend in 08A Electrical Shocks is continuing.
Electrical Shocks and Near Misses 08J across
DOE are now six months in a row above average, one month from an
increasing trend.
SC Program Office charts
are stable.
Electrical Near Miss (08J) did come
in close to the UCL in November, and another month similar to
this in January will cause a trend to be declared.
NA Program Office charts
returned to below baseline for January.
Previously noted trends and
developing trends appear to have ended.
EM Program Office 08J Electrical Near Miss Chart continues to
have an increasing trend, with more than four of five months
have been one standard deviation above average.
In the past six months there have
been eleven events, as compared to only four reports in the
previous eleven months.
EM Program Office 08A Electrical Shocks for
February was above the UCL with three reports.
All other charts are stable on their current
baselines.
Causes A3B2C05 – “Situation incorrectly
identified or represented resulting in wrong rule used”, A4B1C01
– “Management policy guidance / expectations not well-defined,
understood or enforced” and A4B3C11 – “Inadequate work package
preparation” are now tied as the leading cause of Electrical
Near Misses.
