Analysis of EFCOG Electrical Safety Charts
for March 2010
An increased baseline average and control
limits have been calculated for the DOE 08A Electrical Shocks
reports due to the previously noted increasing trend.
Electrical Shocks and Near Misses 08J across
DOE are now seven months in a row above average, a significant
increasing trend.
SC Program Office charts are stable.
NA Program Office Electrical Events and 08J
Near Misses appear to be stable.
NA Program Office 08A Electrical
Shocks have been elevated since October 2009 and will have a new
baseline average and control limits calculated if the data
stabilize at this higher level.
EM Program Office Electrical Events is one
month away from an increasing trend.
The past three months have been one
standard deviation above average.
EM Program Office 08J Electrical Near Miss
Chart continues to have an increasing trend, with more than four
of five months have been one standard deviation above average.
In the past six months there have
been eleven events, as compared to only four reports in the
previous eleven months.
EM Program Office 08A Electrical Shocks for
February was above the UCL with three reports, but March did
return to zero.
All other charts are stable on their current
baselines.
Causes A3B1C03 – “Incorrect performance due
to mental lapse” and A4B1C01 – “Management policy guidance /
expectations not well-defined, understood or enforced” are now
the top two leading causes of Electrical Near Misses over the
past 12 months.
