Analysis of EFCOG Electrical Safety Charts for September 2011

The Overall DOE chart for 08J Electrical Near Miss is showing an increase with August above the UCL.  Also, the past six months have been above average, a seventh month will be a second flag of an increasing trend.  There is a nearly significant increase in 08A Electrical Shocks for September.  There was a nearly significant increase for total events in August.

There were five electrical shocks at SC for September.  This is the highest monthly number ever, and is above the UCL.  Electrical Near Misses (08J) were near the UCL in September also.  This caused a significant increase (above the UCL for September, and also two standard deviations above average for August).

A new baseline has been established on the EM 08J Electrical Near Miss chart.  Originally, a decreasing trend was seen for November 2010 to May 2011, but the data returned to normal.  By including more data (25 months) in the baseline, the decreasing trend was removed, and it appears that the trend occurred due to too few points in the initial baseline.  The total number of electrical events, and 08A electrical shocks remains stable at EM.

The NA 08J Electrical Near Miss has generated a significant increasing trend, with July and August at two standard deviations above average with 4 reports each.  The total number of electrical events, and 08A electrical shocks remains stable at NA.

The top causes over the past 12 months remain the same.  They are

A3B1C01 - Check of work was LTA

A3B1C03 - Incorrect performance due to mental lapse

A4B3C08 - Job scoping did not identify special circumstances and/or conditions

The second and third place causes did swap spots in the chart update.
