Susan B. Derrington, Program Support Planning Manager,
Material & Mechanical Operations, Oak Ridge Y-12 Plant, gave a
talk on "Sodium-Potassium Chemical Reaction at the Y-12 Plant,
Lessons Learned and Corrective Actions". The chemical reaction
caused an explosion which resulted in multiple injuries. This
incident occurred at 9:35 A.M on December 8, 1999. The chemical
explosion injured 11 workers, three of whom required hospitalization.
One worker had third-degree burns on 17 percent of his body and
was in critical condition, received a number of skin grafts, and
was in the hospital for about two weeks. This was a Type A investigation.
Because of the overwhelming response, Susan gave two talks.
Dr. Charles Martin is a Senior Technical Specialist
with the Defense Nuclear Facilities Safety Board (DNFSB). He oversees
the nuclear weapon surveillance program, nuclear explosive safety,
dynamic experiments at LANL, software quality assurance, and safety
of explosives and detonators. He gave a presentation on " Quality
Assurance for Safety-Related Software at Defense Nuclear Facilities"
(Abstract) . Dr.
Martin emphasized that confidence in safe operation of nuclear
facilities depends highly on the quality of computer codes. Software
quality assurance (SQA) is achieved through systematic development,
testing, documentation, maintenance, and execution of various
stages.
