On April 14, 2005 a Limited Scope Accident Investigation Board
was appointed to investigate the Active Vehicle Barrier accident
that occurred at the South Gate of the DOE Germantown Complex on
April 7 and a subsequent incident on June 8, 2005. The report
contains the Board's analysis, identification of causes and
judgment of needs.
On April 29, 2005, a Special Review Team (Team) was appointed to
examine the workplace safety and security programs in place at
the Argonne site. This review was prompted as a result of the
April 21, 2005, suicide of a Department of Energy federal
employee that occurred at the Argonne site. In directing this
review, we have requested the Team to report on two key
objectives: (1) to ensure the effectiveness of existing
workplace safety and physical security programs at the Argonne
site to adequately protect the safety, health, and well-being of
site personnel; and (2) to identify recommendations and lessons
learned from this tragedy for Argonne and for the rest of the
DOE complex to improve existing processes and to develop
strategies for early detection, prevention, and intervention in
the future.
